Been hitting the few spawn points I know of (WS and Barn Bar) and for the life of me I can't get it to drop. Anyone have suggestions on other spawn locations? Thanks

Not quite sure what you mean - in case it's the "Assault Gas Mask", you can get it pretty cheap off of Whitesprings vendors.

There was, or is, a bug with it not displaying when worn. If it's still an issue, please advise.

I think I found mine in or around the shack by foundation but I could be mistaken. Not in the Northwest corner of the map. It (was) one of the rarer spawns, I've only found it once or twice; accidentally ended up scrapping it each time. I blame alcohol.

*Pleasant Valley would be my best bet, too.

I had one for the longest time that wouldn't sell. it may still be in my stash and if so you can have it.

edit: it's gone but I remember where it dropped. The boarded up Slocum Joe's just north of my camp on 59 has a room in the back that has a chance to spawn a random hat.
