Flechettes currently do not benefit from shotgun perks, this seems to be a bug. The two shot effect is also bugged like for shotguns, only adds 1 projectile to the stack.
The harpoon gun is a single shot heavy weapon. It benefits from all 3 "heavy gunner" perks in STR. To add armor penetration use "tank killer" in PER (when using regular armor) or "stabilized" in INT when using power armor.
Also make use of synergies from general damage perks like "tenderizer" in CHA and "bloody mess" in LCK. (+ VATS specific critical bonus perks in LCK if you use VATS instead of free-aiming)
If you do not have enough free special points it is better to use each "heavy gunner" at rank 1 vs. using one "heavy gunner" perk card at rank 3. (Compare the percentage values)
Flechettes used to get good before contextual ammo as you could get more back than you spent. Since the addition of contextual it works the same as single shot, the 'best' mod is the bleed for minimal extra damage.
If you are willing to VATS flechettes could be good with four leaf clover perk to get higher crit rate.
Don't bother with cursed as they break too quick.
Have a Bloodied Commando build with 42 Perception so i know it isn't the right build for the Harpoon build BUT... as i have a huge Perception that gun shouldn't miss a lot but it did. Don't forget that gun has to reload every... single shot which i personally hate so much.
Never ever i gonna use a Harpoon gun again but that is very personal.
I've reported it to bethesda cause it's clearly some sort of bug with the weapon.

My dreams of such melee weapons will remain dreams I'm sure. The video you linked of the game 76's studio was working on originally proves they can totally do it, but not without HEAVY exploit. Better to hope for something more players are likely to benefit from. Besides I've always felt the melee lacked in complexity; not that other platforms are better all around but they certainly outdo 76 in a lot of areas (swinging and blocking, for instance)
From what I understand they'd have to update the engine to bring it anything close to what players want from melee.

But one of the problems with the gun is it doesn't dismember like it did in FO4.
I might be getting mixed up with other games, but if I remember correctly when you dismembered a mob the limb would still act as a projectile. This way you could damage multiple opponents per shot.
Without the 'spike to the wall' effect in FO76 it becomes a single target weapon only... This also limits the usefulness of the flechette mod.
I rarely use one any more but it is quad with reload speed + perk. It is... Okay...
