Mouse movement is steppy but game runs smoothly
I haven't played this for a really long time and just came back to it.

My game runs smoothly at 4K ultra and i lock to 60fps but when i turn with the mouse it feels sort of steppy which i don't recall it doing when i played before.

Any ideas what this might be?

When I first played FO76 (few years ago now I think), the very first issue I had was that despite my frame-rate being a perfect 60, whenever I turned with my mouse, the movement was stutter-y.

The way I fixed it was:
1. Opened Fallout76Prefs.ini and changed iPresentInterval (the built in v-sync) to 0. (File is at C:\Users\(USERNAME)\Documents\My Games\Fallout 76.)
2. Opened NVIDIA Control Panel (Default program if you have a NVIDIA GPU, you'll have to figure out the alternative if you don't), and then Manage 3D settings > Program Settings > Added the fallout76.exe > Changed it's settings for "Triple buffering" and "Vertical Sync" to On.

Hopefully that works for you. And if it doesn't, it's really easy to revert the changes, so it's definitely worth a shot. Don't remember where I found the solution originally. I think it was some small Reddit thread I found after some Google searching.

The last patch seems to have caused some fps issues for some. Review your settings I guess vsync etc.

Someone had suggested downloading you mouse's specific software can help smooth it out if you can't quick fix it. Might be worth a shot.

I've been having this same exact issue. I played this for hundreds and hundreds of hours with NO problems... flash forward to now: bam. Skippy awful gameplay. I don't know what the heck they did to the game but they definitely messed something up. It doesn't play like it used to.

It's not a common problem but it does happen

Ok, it works!
The game tends to, for I dunno which reason, reset .ini defaults.
iPresentInterval was back on 1. Changed it again on 0, with the triple buff and VSYNC in Nvidia panel as explained by LT2483, and works now perfect!

It's nothing to do about the mouse or mouse software, it's all about the DEVs not coding properly... like not even ultrawide support or poor graph options... or more general, it's always been known as a bad game.
Still having this kinda issue so long after released is not acceptable... I bet most of ppl have that very same issue, just not enough affected headache-wise to realize it. But the moment it's solves, anyone would feel immediately how huge confort it is for the brain/eyes!
