did you ever figure this out? I installed the game today and I've tried virtually EVERYTHING. As soon as the bethesda logo goes away I lose audio. I only hear the crackling sounds at the very start.

Late for the party but decided to give my 2 cents since i was facing the same issue.
In my case i had to turn on Sonar to be able to have sound through the headphones. Double checked the sound format, in Windows settings, and only two options were available. It was automatically selected to 8 channel, 24bit, 96000Hz. The other option is the same but Studio Quality. Tried that one just in case but it seems that the game doesn't like it.
I am having the exact same issue. Seems like it started up shortly after the last update. Have tried all the above suggestions and no luck. One thing I noticed is if I go to the in game settings under audio and change the Voice Chat Mode (None, Auto, Area, Team) the sound would return for a couple seconds each time I changed the Voice Chat Mode but then go silent again (all sound except for NPC's talking). I finally left Voice Chat Mode set to "none", closed the game, restarted the game....and now I have sound again. BUT....I have to leave Voice Chat Mode set to none or the sound turns off. I guess it's a good fix if you don't care about voice chat?
I had to create the file and it works perfectly <3
