I play on way lower end PC and don't have too much trouble. Part of the laggy feel I get like low frames comes directly from my connect to their servers. I just changed my connect from wifi to direct connect and I have very little problem now. I still get these weird disconnects and sign in crashes but way less often.
It seems your rig is more than happy spec wise so maybe just part of their "not-so-good" servers?

Hope it gets better for you!

Best wait for someone who has a solution. I know there are some here that do. One thing was capping FPS, but you've tried that. Maybe cap it more. Try 60FPS or something see if that makes a difference.

It's more than likely OP's system is trying too hard. The game is built with the Creation Engine. It's the worst engine on the planet hands down no challenge. It can't deal with high end PC's.

Some versions of windows 11 will have stuttering, its been getting better with time but it still happens.

when i did disable vsync in a ini, the game did feel more responsive and less stuttery for me

Step by Step:

1.) go to C:\Users\YOURNAMEHERE\Documents\My Games\Fallout 76

2.) open the Fallout76Prefs.ini file

3.) Search the ini-file for the word "interval"

4.) set "iPresentInterval=1" to "iPresentInterval=0" (1 = vsync on ; 0 = vsync off)

5.) have fun!

When you disable the game's vsync via .ini file edit you also need to set an fps cap, or you get movement issues in instanced locations.

This doesn't work with win 11, if your running win 11 you just got to deal with it until they get the kinks out.

Don't play in windowed mode, it's bugged. Play only in fullscreen mode.

a friend told me: "you can fix it quite easily tbh by disabling v-sync, putting an FPS cap on and use freesync/g-sync"
"you can tell him to try it, it's not guaranteed to be a fix
but v sync being enabled at the same time as free/g-sync conflicts
and on 144hz its likely it has one of the two
set FPS cap to something in sync range, i just cap mine at 74 for my 75hz monitor... so 144 i would cap to 143"

edit: Open My Documents/My Games/Fallout 76
Open Fallout76Prefs.ini
Find iPresentInterval=1
Change it to iPresentInterval=0

that turns vsync off
if they have high spec and its dropping like that it's likely because v sync conflict... v sync tries to fix it at 60fps to stop screen tearing
which is what they are getting

How would I do a Fps cap?

If you have a Nvidia GPU you can do it in Nvidia control panel and set it individually for each game.

- Go to 3D settings in Nvidia control panel.

- On right side click Program settings instead of global settings tab

- Select Fallout76.exe from the drop down list. If it isn't here you will need to click add and manually find it where you installed the game. Add the exe and then scroll down and set the Max frame rate setting to whatever you like.

If you have a G-sync monitor you need to cap fps at 2-4fps lower than you monitors Hz or G-sync wont do well.

Go into Nvidea Panel and cap fps for the game and also turn off V-Sync and you are all good :)

EDIT: Can see that "insert food here" have wrote it just before i did and he explain it better aswell :)

EDIT2: You dont really need more than 70 to 90FPS in this game though so if you cap the game at 90fps you will see that it will run alot more smooth and specially when you would normal have fps drops you will see it not dropping much if at all as you GPU have spare power to even out.

It is a Bethesda game after all and in FO76 we dont have modders to fix Bethesdas buggy mess.
thank you, I kept hearing about a cap and never knew how to do it.
