- Mama Dolce, spamming one of the items to fix before the server tells you. (fixed?)
- Ever Vigilant, repairing the terminal inside before it tells you.
Sometimes it takes a few stages before the scripts crash and the event/quest crashes.
If I was to make a guess, it would be filling the tub to 60 before before it tells you to. I have seen the tubs instantly filled a few times because people still had items from a previous event.
The script would look something like this:
stage 25 Tell players to fill the tub to 30
stage 30 tub is filled to 30 (goto stage 35)
stage 35 tell players to get more
stage 40 players fill to 60.
Well when you dump 60 in you skipped stages 30 and 35. Then later the script goes "hey wait a minute, you skipped this" and the event fails.
Two other known script skipping issues is:
The astronauts aching back (failed to kill the super mutants at crash site)
Gail not scanning in Vault 74 (Johnny went down from turrets and interrupted door script)
That would be the way to go because you technically do the event every twenty minutes with some server hopping.

I have completed the quest with hundreds of extra acid, but the expiring acid thing is definitely worth testing! Way more plausible than previous theories. Though, I just got hit with that patch update so maybe they hot fixed it...?
The only way it would matter is if there was a check later to make sure there was still 30 or 60 in the tub. Which makes no sense to have such a check because nothing can remove them from the tub.
Although Bethesda has made checks in the past that were not necessarily needed. If you kill Sturges (which needs console or mod) in Fallout 4 then the quest chain to start the molecular relay fails because there was a check in the code to see if Sturges was alive. Dure the check makes sense if doing a Minuteman ending, but not if you are doing any of the other three.
Other events like Encryptid and Radiation Rumble don't have a late-joiner problem so I know the problem can be solved - they need to get on it. Even Eviction Notice and Test Your Metal don't crash with mid-to-late-stage joiners...

I think it's a case of waiting your turn. From my experience is if too many players try turn in at same time causes the bug.
Going off Bored Peons script knowledge it's probably that the game is trying to process too many 'increase count' scripts at once and fails.
I wonder... is the failure rate lower on UK servers because they enjoy forming queues...
If you walk in with 60 acid and turn that in then it skips straight to the part where you turned in 60 acid, skipping everything else.
See it all the time with the console commandos in Fallout 4 going into areas or getting items they should not have then their quests stages are fubar because the skipped stages. Most common is people consoling the wrong Jacob's password item and screwing MacCready's quest in the hospital.
Overall though considering how much script is running there is very little in the game that goes off course. Maybe also be because the server runs the scripting instead of the player's fps. That way the scripting aint running at like 200 fps giving the game like .004 seconds per frame to process scripts.
What I dont get is why the @#$% aint people scrapping them for the acid? Or just scrapping them in general to be carrying less weight. That is just mind boggling, well I know the answer, but the answer aint polite :P
Actually, I would bet it is caused by someone standing at the tub with the 60 and spamming to place it into the tub. Then it goes into the tub before the NPC does their scripted part. Similar to spamming fixing the Mama Dolce's items. Waiting a few seconds to place the 60 after the NPC announces it probably keep it going properly.

The get it done, go back to carpet bombing from the roof mentality and the insatiable thirst for XP.
