Radical Face Mask Cosmetic Removed?

I recently returned to the game after not playing since launch and learned about this cosmetic only mask as a possible reward at the end of the wayward quest line. So I made another character and followed a guide but didn't get it as a reward at the end has it been removed?

Did you side with the radicals? If you don’t. There is no way to get the mask.

Yes I did side with the radicals and I did the +4 charisma option for a peaceful outcome which according to the guide I followed should have let me keep the gun and get the mask.

If i remember correctly, all headwears with effects was removed, not only Radical Face Mask

My memory was the mask was left in game, but the effects were removed. However, that was way too long ago.

I rolled a new character and finished that quest about a week ago. I wasn't even trying and got the mask, so it is still in game.

So for whatever reason I didn't get it. I did read another guide that states you need to either give the weapon or the +4 intelligence choice for the mask and not the +4 charisma choice.

i made a bunch of alts to farm radical mask and cutting fluid recipe for a bit.
Dunno about int or charisma choices, i simply picked the most asskissing/want to join radicals option there is and it gives the mask at the end.
