I would recommend making the full screen setting changes I mention also as it makes the game run better.

Another thing I would try is if you have a high refresh monitor, in Nvidia control panel change the refresh rate to something much lower than the max. Like say 85hz or even 60hz and just see what it does.
for anyone who has alt+enter not actually make anything windowed, use this mod (Fallout 76 Quick Configuration)

and set it to windowed, not borderless windowed, just windowed, and it'll work.
