Launching with black screen and audio
Game launches with a black screen, I can hear the starting audio and all that, but I can't get any visuals. I've tried google's solutions, like alt-tabbing or going windowed, none of them worked, switching to windowed mode doesn't even happen, and launching in windowed didn't fix anything.
The same thing happens with Fallout 4, I thought it was a mod issue at first but I guess not. I tried forcing it to recognize GPU, tried moving drives, tried running as administrator, but nothing seems to be working. Is there an actual solution for this?

Disabling V-sync in the Fallout76Prefs.ini is a possible fix for this issue. It is known to work for Fallout 4 and for Fallout 76. Not saying it will be a 100% solution for you but it has helped other players.

I would recommend making the full screen setting changes I mention also as it makes the game run better.

Did you also try disabling V-sync in Nvidia control panel as well as in the ini file?

Make sure you don't have any other application interfering with the game. I had a desktop sticky notes application that was preventing the game from running in full screen. Don't ask me why but for some reason, whenever I have my sticky notes out, this game and Fallout 4 don't run properly

Also if you have a G-sync monitor you could try running the game with G-sync completely turned off in Nvidia control panel.

Another thing I would try is if you have a high refresh monitor, in Nvidia control panel change the refresh rate to something much lower than the max. Like say 85hz or even 60hz and just see what it does.

i'm on a laptop so i can't do either of those things in nvidia control panel, but i figured them out, no dice.
i fixed it
for anyone who has alt+enter not actually make anything windowed, use this mod (Fallout 76 Quick Configuration)
https://www.nexusmods.com/fallout76/mods/546?tab=description
and set it to windowed, not borderless windowed, just windowed, and it'll work.
