Legendary perk what rads
I dont think it seems to be working. It does not restore rads. The perk card is lvl 1.

Work well for me, don't use anymore Rad-X and Radaway, just use some "ParaRad" and "Brahmin milk" in nuke's zone ... , and I'm at rank 1 for this ...

its very bad described at least in german. it reads in german as if it restores (gives more) rads, but what it does is cleaning rads.

I have rank two of it and with a full set of lv50 T-60 I do not even take rad damage in a nuke zone.

Yeah it is a gradual recovery of Rads and some radiation resistance.
Also note it does not "cure" mutations, though that can be a good or a bad thing. You still have to use radaway if you want to get rid of mutations.

I have a level 2 of the What Rads. I never worry about accumulating rads and they normally go away within a second of my getting them. So I go swimming all the time and normally that is a 2 rads per second accumulation and What Rads removes them within 1 second.

PS. I wear Marine Wet Suit under armor which keeps away water-born diseases.

I'm using a set of secret service armor with max level legendary rad thing, plus radshield and glowing blood pack. Only one item has +25% rad resistance yet. It's around 1000 rad resistance then, and since um some time after beta that means im 200 points worse than a hazmad suit (1200). It needs a fair lot radaway still, but with breathing, and you have a jet pack if necessary, and don't need power armor.

To be optimized (of course there is perks and other items, but i want something simple).

("Wearing" in terms of can use instead of the usual AP and weight reduction optimized wood armor.)
