How does Bloody Mess work?
Is it always 15% extra damage or is it just a chance?

As long as you remember to swap it back in after swapping it out for Super Duper.

The chunk explosion can be a curse or a blessing.

If a hostile above you explodes the chunks can fall to you for easy looting.

Or the hostile can be lost in long grass as the chunks are harder to see.

Either way an extra 15% for only 3 points.

It is annoying to loose loot to the environment I remember in Oblivion turning of the grass as I kept not being able to find a body or dropped weapon. So far in FO76 it has only happened once and I think the body might of fallen through the floor mesh.

When you find yourself chasing a rolling head to loot it and not just looting the body it just cast a curse of stupidity on you.

The chance is like 99% though. And Fallout 3/New Vegas will always have the best Bloody mess. Something about how each individual chunk had its own Falcon 9 Rocket attached to it, propelling blobs of viscera miles.
