Fullscreen scaling, removing black borders at top and bottom.
I think i found a way to scale the image correctly on my 1080p screen to remove the top and bottom black bars without any blur or misshaped pixels after tinkering with scaling apps and such for a while without success:

The game scales with windows DPI scaling so if i set it to 150% in windows it looks like a perfectly scaled non blurry picture to me. Unfortunately there is no way to set a specific scaling when a app starts in windows so i have to set it back to 100% when i'm done playing sigh.

I also recommend setting the DPI scaling behavior to application in the properties of the exe.

Note. If you have a 1440p screen you are in luck since you can use integer scaling to do this perfectly at a factor of 3x, AMD cards and Nvidia cards starting with the Turing series have it in the Control Panel and there are apps like Lossless Scaling (on steam) and Integer Scaler to do this.

I tried this but the sprites didn't look right afterwards. The right eyes are sometimes bigger than the left ones. I would suggest right clicking the .exe and turning on 640x480 mode

This will stretch the sprites which blurs the image a lot because of bilinear interpolation.

I only had the effect with misaligned sprites like you describe if i also had scaling set by system in the DPI settings of the exe, sprites look fine if i set it to per application.

To demonstrate misaligned sprites with per system DPI scaling at 150%, very noticeable on the bigger eye and the picnic blankets pattern :

I had forgotten this a feature that windows has. I like this a lot more than the other method I used. I had followed this guide, https://steamcommunity.com/sharedfiles/filedetails/?id=2872925285 that tells you how to stretch the window to fit to you computer screen without distorting the image but, the issues I found with using the guide were that it didn't connect to the steam servers so I couldn't take screen shots as I played and it didn't recognize me doing achievements like beating a boss but your method works so much better I can still take screenshots and get achievements now I just have to go back and redo the parts I did to get the achievements I rightfully earned
