I have played and completed the game on max difficulty, single playthrough, when the game came out.

I was thinking of trying the NG+ mode they added but wondered if you can just use it as a difficulty modifier and start with fresh/new marines (ie no carryover from previous game )?

No. it needs to be based on a normal playthrough campaign.
And that old campaign needs to be located in the uppermost save slot to work.

But you could just pass days until you get some level 1 marines from events. This would take a lot of days though, probably 50 days or so, and that would also mean you would be sort of rich on materials, engineers and physicians.

and the infestation level would go up with RayThor's method, which would make it a little more difficult.
