Saves are also made every 60 seconds, but a fully completed save will be when you exit -> ‘Change character’ to main menu.

Save the entire directory (there are many different files) so that later there will be no trouble. I recommend periodically saving to another medium (every day or like me every time), if you have two or more of them (I save on the HDD).
When you crate a open world/mission or outpost you are given the opportunity to set it to steam friends only or private, so yes there is single player.
Yes i have seen that has well where it asks you again, but i think it is so that if the host is only one that can play for that day then they can set it to private for that session.
And what is the death penalty in single-player? I suppose like in every MMO there will be time loss by respawning at some designated respawn location, but what about the inventory contents, skills or whatever they are called, etc.?
The death penalty has been removed, but not sure if it has been only remove for open world or it is same for other modes has well.
Edit: you don`t lose progression when dying, you drop a bag where you died and can go and collect it has you will see a death marker on compass, and yes when dying you have to choose respawn and select random location. That is the only downfall for single player has playing coop mode team mates can revive you so you don`t die.
