You can only have one keepsake active at a time and cannot swap back to any previously held keepsakes. That being said, there are many keepsakes whose main bonus is applied quickly and then ceases to matter, such as diety keepsakes and the Cthonic Coin Purse. As such, you can have one of these, get your guaranteed God roll or free gold, then switch again after the boss to get a different bonus.

You forfeit the previous item to pick up the next one, but as many trinkets give you a benefit that applies quickly and stays with you for the run, this is quite useful, allowing you much more control of your Boon collection for example.

There are 2 trinkets which are mostly suited for a full-run use as they gain a specific benefit according to your actions. Other than those, it's either a fair trade or a benefit to swap.

You will soon be strategising your trinket cycle.

I do it now but wonder why so many useless trinkets that give only a chance for better drop from gods. TY

It's not just a better drop, it's also a guaranteed occurrence of that god as the first god boon in your run. This means that if, for example, you want to do a run with Zeus on the Rail (definitely a good combo) then you can take his keepsake and be sure you'll start of with Zeus boon, which along with a few re-rolls almost guarantees you can build it on your attack.

Also - consider taking the coin purse for Asphodel or even Tartarus if you're not picking a god. It's a free 150 coin that you can then still swap out for an acorn when you need it.

I tend to not take a God trinket as my first, and see who I meet - then use the swaps to adjust my starter random build
