Death defiance not working?

Maybe it's a translation error but how I understand it is that it is supposed to give one extra chance per level. But some reason, if I die on Meg and use my token (for example), I don't recover it when I enter Asphodel

Death Defiance gives you up to 3 deaths per run, depending on how much you upgraded it. They can only be replenished through specific means like certain god boons or charon's shop.

Stubborn Defiance is the version you might be thinking of, which offers you one death for every encounter, but heals for less.

Death Defiance is 1 (+1 per rank) chance per escape, as in the whole shebang bottom to top. It can be refilled in a couple ways, I'll leave you to figure those out.

The alternate choice is Stubborn Defiance which is once per chamber.

After maxing it out and getting more experience with the game I find Death Defiance to be better since you can probably clear the vast majority of chambers without issue, that leaves you multiple lives in your rough spots. Stubborn Defiance is great if most rooms are a still a toss up for you.

So it looks like a translation error because in french it clearly states 1 per level, instead of 1 per evasion/run, then 2 and 3 when I will upgrade it

"Level" here means how many times you've upgraded the ability (i.e. its rank, up to 3), not levels of the Underworld.
