35 runs / 20h and no Thanatos. Unlucky or possibly buggy?

Like the title says, in about 35 runs - at least 5 of which entered Styx, 1 of which was successful - I never encountered Thanatos.
Is there a trigger, I have not set off yet, to make him appear or is it just bad luck?

I was well into the 30s, when I first encountered him. Give it some more runs, he'll turn up eventually.

You need to escape at least once just for a chance for him to appear. After that it's a RNG

It should not be necessary to successfully clear the game to get Thanatos to appear. You just have to run into him in Elysium.

The requirements are:
- Have met Alecto or Tisiphone (who can appear after defeating Meg several times)
- Encounter Thanatos in Elysium (about a 1 in 10 chance per chamber)

Subsequently he will able to appear in Asphodel or Tartarus as well. (Note I'm spelling out the requirements this directly only because of the problems being reported here.)

The 1 in 10 chance is purely random so it's possible to get unlucky over long periods of time with regard to his initial appearance. 30 - 60 escape attempts is well outside the bounds of how soon we would like for players to encounter Thanatos, provided they've been able to reach Elysium, so we will be looking at ways to make his appearances more consistent (without being methodically predictable as we do like some randomness on this).

Ah, okay. Thanks for the in depth clarification.

I have fought Alecto and Tisiphone a couple of times and have reached Elysium at least 10 times. 5 times to Styx/Endboss, so all the way through Elysium. Dying midway through Elysium I would estimate maybe 5 possible chambers for him to appear per run to Elysium, is a chance of 0.9^50 to not appear - so about half a percent, or 1 in 200 players will have this rate. (If my math checks out. It's very rough estimates, though.)

Forcing the first time wouldn't be terrible. Perhaps make it assured to happen in the room before Theseus if you have defeated Theseus in previous runs. Again, for the first appearance only.

Hi, it's been over a year now, I started the game a couple weeks ago, and I've gone 31 runs with only encountering Thanatos once and died during that encounter, since I was very low health going into it. Since meeting him, I've been through Elysium several times and I've gotten to fight Hades once (haven't won). Is this still just my terrible RNG luck?

It took forever for him to reliably show up for me. By then I'd progressed the story/side character arcs so much that when he did show up at home he had a backlog of other npc interactions before I could have a chance to swoop in and flirt with that yellow eyed putz.

ya try not having cerberus after talking to him 500x beating the game and just giving up after 50 runs after the true ending.
