I think it means harder challenge but also better reward like getting two keys instead of one, can be wrong though.

Once you beat the game the "Pact of Punishment" and "Heat" mechanic are enabled. Basically at the start of your run you get a list from which you choose modifiers that make your run harder. Then the boss rewards get reset (you can acquire them once more instead of getting more Darkness) on each "Heat level" for each weapon.

The skull means an elite fight, for exemple the fight against Minotaurus when he is alone is one of these. In the first floor it could be the 2 armored bombers, the 2 armores skeleton that send plasma waves to you in floor 2 the boat is considered an elite fight as well and so on. They are scripted so after a few runs you know exactly which elite encounter it's going to be.

How about the rewards, though ? Do you know if you have increased rarity for boons, for exemple ?

You can acquire Titan Blood in the following ways:
1) Beat Asphodel boss for the first time
2) Beat Minotaur + Theseus for the first time and then exchange the Ambrosia for a Titan's Blood at the cafe vendor
3) Beat the final boss for the first time
4) Exchange gems -> keys -> nectar -> diamonds -> Ambrosia -> Titan's Blood at the cafe vendor

The first three can be repeated for each weapon at each Heat level (you unlock Heat levels after beating the boss for the first time).
