why do heads not get blown off anymore?

Everytime i shoot someone in the head with a shotgun its replaced with the same head model that just has the front missing. Is this a bug or what?

Because it was silly beforehand. You'd hit someone with one pellet and their limb/head would just disintegrate.

more silly now imo, now everyone head is replaced with a generic african american mans head thats bald. Even if you shoot it a second time it does nothing.
