how to access cqb training mode?

i unlocked the training facility but have no idea where/how to start the mode.

I'm not sure entirely what you mean, but the training facility has the shoothouse. I haven't explored much down there, but I think the timer starts either automatically, or there's probably a button near the main entrance to start it once you enter.

I think automatically, when I walked through I remember hearing a buzzer once I left.

That is a modded map, one I quite like, though I will try to see if I can remember the name.

From my quick Googling, it seems the author has delisted it. Either to repair it, or because they don't want to update it. I suspect to update it.
