I've been doing the missions one at a time solo S rank and am at 13/18. I'm sure duo Carriers would be fine.
Virtually all of my Solo S ranks end the same way: I have 3-4 body parts red-dark red from doing the entire mission solo and catching the odd stray, then the enemy that has been screaming at me but only repositioning when I close in inevitably shoots me through a wall/door and one taps the last of my hp.
The rest of the "virtually" is taken up by missing shotgun shots and dying of my own doing.
I was actually thinking about how to do the ironman without your AI dying and all I came up with is replacing my non lethal shotgun with a lethal shotgun and blowing away anyone who gives me the slightest resistance.
However, I agree that AI + coop should be a thing. The way it should work is that at 2 players specifically, teach player should control one team. First player gets blue team, Second player gets red. This would be insanely fun as an ironman mode. It would also alleviate some of the ammo issues.
I like that in SP I can load my AI up with wedges and stuff like that, and save gas/ammo for my own guy.
