Is the implementation without fault? Absolutely not but it's actually pretty good for what it is.
The suspect AI however....
Swat 4 ai reloads their weapons at under 25% ammo, yours don't.
Swat 4 ai doesn't laser sudpects that are in the process of fake surrendering, making any form of reaction a joke.
Swat 4 ai doesn't commit high amounts of friendly fire due to overpen.
Swat 4 ai doesn't 1 tap suspects straight on the head while clearing.
Let's not talk about suspect speeds/ai.
I've often seen my mates just running blindly into enemy crossfire, doing their own thing, sometimes running around in completely different places instead of staying with me and once I even had the situation where a mate blindly ran shortly after I gave the command the room to be flashed by a flashbang into it, only to get shot by multiple enemies. I've already shot down a mate who suddenly ran around in the middle of the room in the crossfire between us and the enemy instead of staying behind the commander or at least close by. I also noticed in the crack house missions that we got shot from outside the building as we was running around in the second floor. In addition to that the officers shouted to enemies who standing behind a wall in that small house with the cave, we were outside.
Doesn't really make sense.
But I agree with your point that there should be inaccuracy, firing in the same direction without necessarily immediately hitting and also occasionally shooting when the suspect is already dead.
