It can be inconsistent yeah, probably because of early access. It'll likely get tweaked. Was the guy on the other side of the door armed? The runner was likely because they were armed, the game doesn't handle it so great atm because of how aimbotty the AI can be

Yell first, and see what they do. If they try to raise the gun, then shoot them. Just holding a weapon is not enough. It was bugged in the first build, where you could shoot anyone with a gun, as long as you didnt yell at them. Was not intended
You dont have to wait for them to shoot you to shoot them, you can shoot them if, after telling them to surrender, they motion to aim their gun at you, you dont have to wait for them to actually fire.
