You probably already know about this, but the original game has a hidden "special" stat, which prevents you from getting too many powerful items in a single run. It increases every time you pick up a "special" item, and drastically increases the chance of other special items being rerolled into more common, less powerful ones.
So, if you want to find SMB Super Fan, your best bet is to play as Isaac, use the D6 in treasure rooms and avoid picking up special items.
To find out which items are special and which are safe to pick up, use Platinum God[platinumgod.co.uk]. The majority of familiars, damage ups and tear-changing items are special. Brimstone (don't ask me why) and all active items are not.
