Try a new game with the cheat enabled from the start, if it does not crashes then that's it but it would still be weird imo.
https://github.com/libretro/libretro-database/tree/bbab349e5e3884b0f6386ba76bb193b648b50847/cht
The reason why this happens is that your game version is not compatible with cheat file.
Because these codes are actually the instructions with memory addresses. If your game version is different then most likely the values cheat supposed to change are on different address, and the cheat will change another, random thing, which will lead to random things, usually glitches and crashes.
Use this site for codes:
https://gamehacking.org/system/psx/silent%20hill
Pick your version. Or download another iso for which the better codes are available.
Then download .cht file. Make sure you've picked "libretro" file format.
If you have some exotic version, for example, of another language, then search for this exact version.
Or, you can always hack it yourself using RetroArch tool - at least numbers and gaguges are quite easy to find and change/freeze.
