This is further established in December, when the fog from the midnight channel leaks into the real world, causing many citizens to suffer from medical problems, fatigue, panic, and paranoia.
Because the fog is stronger in the midnight channel, and because Nanako is only 6 years old, it was probably especially hard on her, which is likely what caused her death.
That being said, I have no idea how Teddie ends up bringing her back.
They mention this very briefly when Teddie comes back but basically a doctor says some kinda BS about how the voices of Teddie and everyone else were heard by Nanako which made her keep on fighting to stay alive even though her heart had already stopped.
