Wheres my Insane Achievement?

I went through hell and back. I retried so many times, took small breaks, got one shotted by many things, and I didn't get my Insane achievement WTF i did every mission on Insane. I went into my campaign and when i scroll through the missions it as the insane symbol and I did all that ♥♥♥♥ on SOLO
I did all this with no help and I don't get my achievement!

Maybe contact support on their forum or twitter? I haven't tried myself, so I can't confirm whether it works.

Happened to me too. I got it by going by checking my Stats. Press 2 when you're in menu and check your stats. Click on one of the Acts and it should say what difficulty you did it on. If it's blank then that's the Act you need to do again. If you go to the Campaign lobby and select chapters It will say that you did it on Insane on solo for every chapter, but your Stats will say so otherwise. If anything, do it offline. Your collectibles won't save during online so I'm guessing playing offline should track your stats. Going online will lose track of your collectible but steam will update, so just remember which collectibles you already picked up. Trust me, I had to do Act 2 on insane all over again.

I also didn't get my achievement for completion on any difficulty or insane (completed insane, stuck at 86/100 on both)

wow really? I'm glad I just took a break then because I'm not gonna keep going unless they fix this.

Figured it out, if anyone else has this issue in the future, make sure you check your "Stats" on the main menu page at the very bottom. Act 3 Chapters 2-4 did not count as beaten on insane for me due to server bugs although in chapter select, they showed as finished in insane, they didn't under the stats. Went back and replayed them, got the achievements after I completed Act 3 Chapter 4.

Edit: I see Leogriff already pointed this out, I'm just copying and pasting to different threads.
