I mean, I really wish there was a way to practice this in a safe environment that is not the chapter 1 tutorial.
for me, even on lovecraft difficulty the door minigame lasts on average 3-5 seconds.
I guess that was my problem.

I'll keep in mind to slam the door every so often to keep it closed. Though I do try to get as much strength as possible. I guess that is also one of my problems. I had trouble keeping it closed as well. Though I'm currently palying as Erica on chapter 2, who has low strength.
No, while pressing A or X like crazy, a circle will be created around the trigger notification. The more power you got through button mashing, the more force you put into it.
In general, I advise to slam the door when the gauge is either half full or when the arms are reaching out of the opening.
But even then I didn't really get the hang of it. Especially with weaker characters, like Erica.
When you need to PUSH the door, you can see the Presence's hands/limbs latching on the door. If there are a lot of hands/limbs on the door, your character will be killed soon, so they are pretty much a visual indicator. Other than that, you can see your character backing away because of the Presence pushing through.
However, when you need to PULL the door, it gets harder since you can't really tell if your character is winning or not. So, look at the amount of hands on the door and go from there. You can also use the camera as a guide. If the camera is zoomed into your character, they will get pulled away, so try and keep the camera away. Aside from that, you can look at the character animations. If they are struggling a lot, you know the drill, they'll die.
