How to deal with the Door QTE?
Obviously, I'm doing something wrong as I have died to every single door QTE so far.
Or maybe I misunderstand the mechanic behind it.
So far, I mash the button, then use the slam option, if the door opens too wide. Does slamming the door accumulate to a hidden score counter to shut the door or does it reset the Presence? I did notice the bar fills up at different speeds after every slam, so I assume I need to slam when the bar is filled to the max. Or is there a hidden timer and I just need to outlast the Presence?

You need the door to not open. Gather as much strength as you can and push whenever it starts being wide open. You don't need to fill the bar.

Is there a visual cue when the door has opened too much? I still have trouble.
I mean, I really wish there was a way to practice this in a safe environment that is not the chapter 1 tutorial.

as long as you don't spam the slam button you'll be fine, spam the push key as fast as possible and press the slam button after every few presses of the other. you can win without ever even getting the bar to half so long as you slam it enough to keep the door closed.

for me, even on lovecraft difficulty the door minigame lasts on average 3-5 seconds.

I'll keep in mind to slam the door every so often to keep it closed. Though I do try to get as much strength as possible. I guess that is also one of my problems. I had trouble keeping it closed as well. Though I'm currently palying as Erica on chapter 2, who has low strength.

No, while pressing A or X like crazy, a circle will be created around the trigger notification. The more power you got through button mashing, the more force you put into it.

In general, I advise to slam the door when the gauge is either half full or when the arms are reaching out of the opening.
But even then I didn't really get the hang of it. Especially with weaker characters, like Erica.
Aarrrrh. When I see the door opening so wide, I trying to gather full circle of strength and pushing RT, but I always dying

Less is more and half full has certainly helped me a lot. Even on Lovecraft difficulty.
Gathering power to half of circle and pushing RT - I'm drying anyway.

Since you asked for a visual cue for how much the doors has opened, it's pretty simple.

When you need to PUSH the door, you can see the Presence's hands/limbs latching on the door. If there are a lot of hands/limbs on the door, your character will be killed soon, so they are pretty much a visual indicator. Other than that, you can see your character backing away because of the Presence pushing through.

However, when you need to PULL the door, it gets harder since you can't really tell if your character is winning or not. So, look at the amount of hands on the door and go from there. You can also use the camera as a guide. If the camera is zoomed into your character, they will get pulled away, so try and keep the camera away. Aside from that, you can look at the character animations. If they are struggling a lot, you know the drill, they'll die.
