Anyone know how to get this achievement? According to guides, you need to collect all 8 documents in episode 5.

I have done that twice and also did the Old Photography document which made up 9 documents in total.

Got that achievement on my very first try - don't remember how many documents I had.
I just remember I never got the photograph, because it was never added to my inventory and I'm positive I did it right.

For all I remember, you need to collect all documents that were tied to the Asylum and Ariadne. If I recall correct, this includes the three documents of Berenice and at least one document from the director of the Asylum, located in his office. And you also need to inspect the backside of at least one document.
I'm not sure if Ariadne's pictures are necessary.

This is really weird, I'm actually playing on console so it might be bugged here.

I have done this twice, inspected all documents front and back as well, still nothing.

Just want to confirm what the Developers have said. You need to start a BRAND NEW GAME from the VERY BEGINNING on H.P. Lovecraft difficulty. Playthrough until chapter 5, then collect AND READ the following documents: Taxidermist instructions, Poem by Berenice, Letter from an artist, Third experiment report, Doctor Alan Cedrac`s note, Dismantling notice, first experiment report, second experiment report. NOTE: The photograph document after you solve the picture puzzle is NOT required.

Complete chapter 5, then finish the finale and the trophy/achievement should pop during the final cutscene. There are many guides online that say that you just need to change the difficulty at the end of chapter 5 or at the end of the finale. THIS WILL NOT GET YOU THE ACHIEVEMENT/TROPHY. YOU MUST PLAY THROUGH THE GAME FROM THE START.

My advice is to use backup saves/cloud saves. Back them up after you complete a short block of progress if you are struggling.
