1) The crypt is question is the one that's really obvious and is interact-able. Daniel says something like "I'm sure it's locked".
2) No one needs to be dead to get it.
3) I don't know if it matters where you enter from, but mine triggered when I entered from the bronze-key-door with all the small statues in the back of the cemetary.
4) It triggers when you get near the door with a mini-cutscene.
5) Once it opens, you run to the bottom down the stairs and pick up the item like a normal item (e.g. "Do you want to pick this up?"). Then you have to run out. The timing seems to be pretty generous but don't screw around.
6) It's probably available all the time, but it's definitely available when you've done everything and have all 4 keys and have 4 of the tarot cards.
It probably took me over 20 or 30 minutes or just running back and forth between the two exits for it to work. I was about to give up for the night. Pretty dumb mechanic.
It will be the same for PC as well. You will only have one chance of going in when it opens, where you either successfully get the confession, or get locked inside and die.
