Palace of Ice - Triangle Key

I cannot move forward with the game due to what appears to be some manner of sequence bug.

I got to the point where the game wants to to insert the Triangle Key in the Telema Ruins, and upon doing so, the quest is not updating.

I can interact with the Minor Gate, and was able to complete the Optional Objective. However, the game does not recognize that I have The Sovereign, so it is not allowing me to teleport out of the Zone using the Long Distance Teleport.

I'm stuck here as well, will not accept my triangle key, keeps telling me to ' read the instructions ' which I have many times.

Same. "Read the instructions." Why does it even check for that?
