How do you get it? Please help

I did not manage to fail the D'Angelo quest line, despite being as rude and unhelpful as possible... The other three quest lines are much easier to intentionally fail.

I suspect that you can only fail recruitment during the first encounter. For D'Angelo, here's what I did:
- attack him in his office
- when he asked for my opinion about the crime scene, I said "you're the detectice, you tell me"
- I deliberately misinterpreted the evidence and suspected the murderer to be a Tremere
- when he asked me to quietly get Larson's attention, I instead fed on the bywalker and caused a ruckus
- I insulted and threatened Larson whenever I had the chance

After that, D'Angelo stepped in, handled the matter with Larson in his own way, assessed my performance throughout the day, concluded that it's "not gonna work", and vanished.

It seems that you may not have tried hard enough to fail. ;)

Indeed. I tried harder after posting and managed to convince him I have no detective potential. I pretty much did everything you mentioned, plus I was enough of a fool to ring Robert Larson's doorbell.

Still, it's much easier to displease Hope, Agathon and Tamika than D'Angelo. Maybe the Nosferatu have lower bars for social interaction... ;)

Sounds plausible, but I guess Kaiser would disagree, at least as far as kindred below his status are concerned. ;)

But you're right. The quest lines for Agathon and Hope can be very easily failed by just saying "No" and leaving. And for Tamika, you just have to be a very obvious ♥♥♥♥, recognize the cue that you "overplayed your hand", and then double down on it. With D'Angelo, it's clear that he's testing you, but his "scoring system" remains opaque.

Easy. Be an obnoxious prick. Pick the worst comment every chance you get during your first meeting and she'll reject you quite clearly.
