Are Luskan, Waterdeep, Baldur's Gate or Candlekeep on the world map?

This is the Neverwinter MMORPG .... not the "Forgotten Realms" MMORPG. Basically, what you have is the City of Neverwinter and various places on the Sword coast and elsewhere, but Neverwiter is the only major city.

I think the game would have been much more popular if it was set in the entire forgotten realms... not just neverwinter.

Well, its not set just in Neverwinter, in fact a small amount of the content takes place within the proverbial city limits. It does contain the major hub for vendors and interacting with both the game and other players. The game also sends you along the sword coast and even to some extremely remote places, but yes Neverwinter itself is the only major city. Caer-Koenig is a small town and the only other player hub you can visit, and of course you have guild strongholds.
