Worth to buy it only for Kaycee mod?

No. The game as a whole is worth it. The Kaycee mod is only an extra post-game challenge if you're left wanting to play something after the main story, which by the way, is brutal. Of course Kaycee's mod is also a very good mod because it tests your gaming ability, and your luck, now more than ever

i don't care for main story i wonder how much fun ill get from kaycee's mod and how long it takes to beat it as whole.

It depends on how much it costs you to finish it. A few hours might be enough if you are sufficiently trained in the game. There are people who find it much more difficult or even leave it halfway. In my case it took me a few hours, and the final challenge, a couple of days. It's quite fun but don't expect the game to be giving you new things all the time. What you see in the first act of the main story is what the Kaycee mod offers you, plus some additions like new cards, decks, nerfs, special challenges, and more. If I'm honest, for me, it wouldn't be worth it because I like the story as well as just playing. I also get tired quickly of repeating the same thing over and over again when there are few options, and there are few cards in Kaycee's mod. But hey, this is my opinion, I can't tell you whether to buy it or not, but I can get you a little closer to what the game offers you so you can decide for yourself.

I don't know Sly the Spire, but between that game as a whole and Kaycee's mod, I'd choose the former. If I were going for full Inscryption I'd pick this one, but since it's just Kaycee's mod I wouldn't pick it.

hmm is game as a whole really good? i got pretty scared of main storyline because of reviews that it's unplayable etc

I finished it all without problems, and even platinumized the game, and I can tell you that it is one of the most fun and unique experiences I had the opportunity to play. Don't get carried away by the criticism, also, to play the Kaycee mod you're going to have to beat the story mode anyway hahaha. I can assure you that you are going to enjoy that game. I would like to tell you more about it but I don't want to ruin your experience if you play it.

Not really because KC mod isn't that rich in gameplay in the end. It gets harder, but the further you progress the less freedom you have for effective decks and in the end you need a defensive infinite and/or a super aggro deck which means relying on RNG to give you cards/totems/items/locations with correct synergy (the initial pelt trade usually sets your objective).
