Heart to heart time. Try talking thru all his options in case that's a new trigger?
Managed to get Johnny to appear somehow by doing something different. Don't know if it'll work for anyone else but here's what I did-
My game is modded to heaven. So, deleted every mod and started an old save at Transmission. Played until I got the Chippin in quest. What I did different this time is, I didn't track the quest. Went straight to Afterlife and boom, Johnny spawned.
For testing purposes, I loaded a save prior to getting the quest and tracked Chippin in this time and the bug appeared again, not Johnny. I think tracking the quest messes something up. I tested again with mods this time and it worked all the same if I didn't track the quest.
TLDR; When you get Chippin in, don't track it. Go straight to the Afterlife.
If someone gives this a try, let me know if it works :)
EDIT: Forgot to mention something. I did not fast travel to Afterlife. Simply drove a car there.
