Can't save after (spoilers)
I was doing a new playthrough before Phantom Liberty, and I just reached the part where you have to kill Placide and leave the church with the his key. After stepping outside and going through the following story scenes, once I can move around normally again(after chatting with Johnny and getting the dog tags) I can no longer save the game.

It's been a long time since my original playthrough so I don't remember if this is intentional or not. Also, at that point Hanako calls and for whatever reason I'm unable to answer the call or call her back, even though that is the primary objective being displayed. When I call her back, it just rings a few times and then the call drops.

I feel like this call issue could be intentional to force the player to go through the scenes at Afterlife with Rogue that have now become available, however I feel like, even if that was intentional, it wouldn't explain the inability to save.

Does it say you can't save here, or does it just not work?
There are places where it goes for a long time without letting you save, but it always says you can't save when you try.

That's accurate, it says that I can't save at this time, and it says it wherever I go. Even after doing other random gigs and resting at an apartment for the night. You're saying this is presumably intentional? It persisted after going out drinking and finishing the meeting with Rogue, I hope it's intentional and not a bug, but I can't figure out why this would be the choice.

I heard going back to V's apartment in MB10 and sleeping will fix it, so might be worth a try.

Personally I only had this bug happen once and it was right after Judy's Pisces quest. Luckily I caught it pretty quickly so just reloaded the save and haven't had it happen again.
The way I found to fix this is going to another section of the city way apart from the location you are now. This has happened to me a couple of times on different quests so it may not be related to a specific quest.

using the fast travel network also helps

I tried doing that, and I wasn't able to sleep there unfortunately. Here are a couple pictures of the problem. [imgur.com] One is showcasing the inability to save, while the other is of the strange phone icon it showed when trying to sleep. It might be worth mentioning that I'm also unable to use fast travel. I've tried loading before this point in this save, and every time I go through this story moment I end up with the same problem. The phone icon seems to imply that I need to make the call to Hanako first, but I can't answer her call nor does my call to her go through.

Here are a couple images that show more of the issue.[imgur.com] Normally when getting calls I can press "T" to answer, but for this call I don't get any prompt. Also, none of the fast travel stations work. This problem starts in Pacificia and in the screenshots from last post I had shown that I wasn't able to sleep in my main apartment in the first Mega Building 10, so as I had to drive all across the map for that, it appears as though going to other areas of the city doesn't resolve this issue.

This is a known bug that can happen after completing a main story mission.

To fix it, you need only reload a save. Ohh, but you can't save... so how do you do that without having the replay from last checkpoint?

There are two good ways to go about this, the first is to interact with your stash. Just summon one of your cars/bikes and have a look in your stash. Once you exit the stash the game will create an autosave, which you can then reload from.

The other method is to interact with a vendor. You probably picked up a bunch of junk guns and items durring the mission, so just drive to the nearest clothing or weapons store and sell off all that junk, and the game will create an autosave from the point you exited the vendor screen.

Now just reload the autosave you've just created, and you'll be peachy.

And yes, they really should to fix this, this bug has been well known and reported since launch.

I had unfortunately already tried reloading, but here is a shot[imgur.com] that shows a presumably related issue. I can't open the stash to force an autosave because the game thinks I'm in a call. I'm clearly not in a call, I wasn't able to answer the previous call, and if I try to call Hanako it just rings and eventually fails. There was a different attempt where I was able to get into my trunk and save/reload, however that didn't fix the issue of being able to save normally or call Hanako. Also every time I load that save I immediately get called by Hanako and can't answer, afterwards my phone icon is red and unusable.

I think I'm just going to have to keep going further back and playing through this story section until one version isn't bugged.

Sadly, that might be your only option.

Phone bugs, including the recipient never picking up, or the call never registering as ended, have also been reported before, but i'm not aware of any surefire way to fix that. I'm not sure anyone actually found a fix.

The only thing i can offer is this: Mission bugs like these seem more likely to happen when you're running low on RAM/VRAM, atleast i have found that i am more likely to encounter them if i do a main mission after having played for a while, versus if i do that same mission on a freshly booted game.

So yeah, in your shoes i'd find a save just before starting that mission, and then trying it on a freshly booted system (maybe even kill some background apps just to be sure). Best i can offer you i'm afraid.
