There's an 'enable secret ending' button under the 'Quest Facts' category in it.
First backup your save files tho, and don't tweak any other stuff or you might cause weird bugs.
In the oil fields conversion, you have to choose
"The guy who saved my life" when Johnny asks what you would put on his real headstone.
THEN
"Nah, you f*cked that up too'
"What do you WANT from me?!"
"Okay, but as second chances go, this is your last"
Watched a guy get the ending with only 40% affinity with him in his YouTube vid a while back, and he was testing.
Used to be, you needed to have 70% with Johnny before around patch 1.5 or 1.6 but it was changed to only need this set of choices afaik.
Might also want to choose
"Yeah, yeah I would" at the convo in the Pistis Sophia hotel when he asks if you would take a bullet for him, I know it used to just alter his reaction to your choices inside Mikoshi, but it might carry some weight now.
Again, this is pre 2.0 and wiki pages sometimes never get updated.
