I had the same exact problem... follow the post I made here: https://steamcommunity.com/app/1091500/discussions/0/3875967397716463055/
I haven't crashed once since applying this. Make sure you check it stays applied and re-apply as necessary if you restart your PC.
Expression: EXCEPTION_ACCESS_VIOLATION (0xC0000005) is ALWAYS a driver issue.
Sorry, but I did a fresh install of Windows and still got this error with a clean install of everything. It's already been noted the fix I posted is what resolves it. You need to turn your cores down manually.
What do I do if I am using AMD CPU?
His method is for ppl with Intel CPU, not AMD CPU
Then at one point when I was quitting the game, the game crashed so badly that the PC restarted. After that it started crashing again with the Access Violation error, always shortly after sarting up and loading a save. I wiped the DX12 cache again and since then no crashing again.
So I think that is the problem, which can get hidden in all the other remedies you try.
I am also experiencing this just today. Not sure about the mod, but how to remove it when you can't even start the game?
Sadly, this only worked for awhile, now my game is crashing with this error now, at random times:
Error reason: Unhandled exception
Expression: EXCEPTION_ACCESS_VIOLATION (0xC0000005)
Message: The thread caused a user-mode data execution prevention (DEP) violation at 0x46E68FF6F0.

Thank you very much. After using the Intel (R) Extreme Tuning Utility you mentioned, the problem was indeed solved. The multiplier of 13900k was adjusted from 55 to 53 or 52, and these large games can run again without any errors. Thanks again
Download the "Intel Processor Diagnostic toolkit" and have it run the tests on a loop. My system consistently causes a BSOD after a few minutes, so I'm going to request an RMA from Intel. It just seems at this point that it is the most likely cause for all of my crashes and other strange stability issues.
