The only exceptions I know of are:
One of the boxing quests that take place in the same building as a Muamar Reyes gig that could result in the person you're meant to fight accidentally (if you went in guns blazing) getting killed.
It's not really time based, but you can accidentally miss it and soft-lock yourself out of the rest of the boxing quests.
A side job where you can repair the roller coaster for some people that disappear after you do the story mission Double Life for Judy Álvarez.
The roller coaster itself can still be repaired, but there won't be any map markers to tell you how or the people there to cheer when you do it.
Side note about that location, if you go anywhere near the ticket office have a piece of catfood on you.
that one, you have 24hrs to turn up to help her, or you will fail.
it's best to just drive over and sort that one out tbh.
there are a few other quests that can be failed, or time out.
the monk quest can dissapear if you don't do it when there is a marker for it (found that out myself in my current playthru lol. i left it too late into act2 and it's marker dissapeared. but that can be done in act1 or the start of act2)
there is a marker in Watson for a guy whose implant is busted, and he needs to get to his ripper asap. you must be driving a car, or summon a car. cant use a bike due to him ah.. being unable to sit properly (which tells yoh what implant failed XD)
the quest called 'happy together' can time out if you agree to talk to Barry but wait too long, or wait too many hours after the first time you knock on his door. it will fail if you wait too long.
(best to knock, do a days worth of stuff then try again. i go visit Mama Wells, then try again after getting back from Haywood)
you can miss a quest up in 6th street teritory, just below the dam area if you don't go around there. but its got a decent pop radius since it's aparty with loud music and folks shooting guns off (typical sixers XD)
and there is a hidden quest you can stumble on, that starts in a garage. so always check for the doors you can open (some nees a strenght check, but gorilla arms can help there)
if you helped Meredith Stout, you can reply to her text message and meet up with her. but if you dont want to meet her again, just ignore the text. dont reply *at all*.
It is certainly debatable as to being a feature or a bug, as the devs keep changing the pathing to do this, but after a few "modifications", they haven't made it impossible, only different.
Jackies funeral has alread been mentioned.
Scorpions funeral can be missed, too.
And there is the mission you get from the barkeeper in the cojote cocho. Times out a day after you get it (which can be an issue, since it says you should follow her during the day. So if you pick up the mission in the afternoon and think "ah well, I'll do this tomorrow", there is a good chance you'll get there too late).
As for 'Riders on the storm': If you wait too long, Saul dies, and Panams questline fails. This will make the Star eneding unavailable
In general missions dont fail if you dont start them (the exception is the already mentioned fight against Rhino) but there are some that fail if you start them and then ignore them
