There are many customization mods, so many that I have to install about 500 lol when the game decides to release another upgrade
Realistically, with the crappy food they eat in night city (a caricature of modern America, but even more corporatised and even more ZFG with regard to people's health), people would probably be even fatter than they are now.
That being said, there are really not that many cars and people do seem to walk around a lot more, so that might offer some lore explanation for the low number of obese people. Although it could also be that the fat people are indeed just ridiculously more fat and so can't actually get up and walk around, so you just never encounter them. Got a vending machine, a TV, a couch and a toilet within easy reach. No reason to ever go outside.
Look what happened to Dexter Deshawn. Dude couldn't outrun Takemura because he was carrying around all that extra flesh.
but... "Dexter Deshawn's 300 pound ass" probably wouldn't work for V, given all the things they have to do to rid themselves of Johnny.....
Sneaking around with Songbird? i don't think Dex would fit in those air ducts.....
I am surprised you can't be an "Animal" though, since they're quite prominent across the game. Acting as Guards, Bouncers, Mercs, even in Dogtown. Could have been a new starting life path; Trying to get out of the Animals. (There would be a few quests across the game where such a life path would have been useful, in both base game and DLC)
Maybe a thin/average body type choice could work lore-wise. It will never happen though, since every single clothing item, weapons, animations will need to be done twice for each body type to avoid clipping issues and so on.
And no I am all for it if they could pull it of. I think one thing most large games always get wrong are the people in it. Most times same height posture etc but technically I understand why they do it and if that means choosing between development time then height and width options are a reasonable thing to throw out.
I have done clipping fixes for some games for myself like body-slide outfit studio in fallout 4 and it is tedious if you want to have it look good on everything. A shirt needs less clipping fixes then a body armor for example or your ending up with a body-warmer instead of a shirt regarding thickness. Sometimes you instead have to cheat by reducing the body instead of the clothing because if something becomes to wide on the side it might start clipping through the arms for example and that is even without calculating in clothing physics. Basically to do it right you have to create separate models, textures, animations, physics calculations for everything.
So yes I think it is absolutely to do with the amount of work. Maybe in the near future machine-learning can resolve it partly instead of it involving hours of manual work but we are not there yet.
Vulva options are available via mods. I'm pretty sure I've seen mods listed with those at Nexus.
Gymfiend is what you are looking for then.
Be as beefy as the Jackster, or River Ward or Placide.

There are a lot of clothing available for this body type too, although most clothing is custom made and you need ArchiveXL and TweekXL to get them to work.
Lots of tattoos and even custom body cyberware too.
Might as well load EquipmentEX along with, so your gym-rat can layer that clothing.
(Might I suggest Kwek's Driving gloves clothing mod. Adds fingerless gloves in a fair few colours. Good for sweaty palms issues when lifting weights)
And.... If you want all that muscle to be a Hustle (Hired Muscle) for someone in a gang (yes, there are Valentino tattoos available for Gymfiend) you could install the Gangs of Night City mod.

The character creator is indeed even with extensive modding, still not complete... but this cannot and will not be changeable :s
