Outside of lore, hacking in game refers to the mini-game you will play once you have jacked in. Your number of attempts is displayed as empty boxes at the top, and you select from the grid. The highlight will show you if the next move will be a column or a row, so hover over a number and you'll see which. Therefore, you have to select one in a column, then one in a row, then one in a column again, to complete the sequence.
