Regarding "Balls to the Walls", I chose the Aldecaldos option both times I did that, and got the post cards some time after driving them to safety. But I don't remember anything about beheading? Can you find him beheaded somewhere in the game later, perhaps?
And good tips about the fixes for the missing tv-audio. Been using those since they were released. Wonder why this hasn't been fixed 'officially'. Maybe there is some censorship involved, or something? That they're no longer allowed to keep those 'news-stories' in the game? No idea, though.
The easiest way to "start" the Highway man is going to the dam area (where you do the drink / shoot quest for the 6th), on the south eastern corner on the hill is a building with a police car parked in the corner. There's a garage door to open.
You can find Paco's beheaded corpse across from Dogtown entrace, on the upper area of the NCPD building, an iconic sniper lying beneath him.
If the news weren't allowed to be kept ingame they would have taken them out entirely. Probably a QA oversight when fixing something else. Thankfully, fix works.
I don't know of any radio fixes but if you do, let me know.

I don't use the radio much, just check from time to time when driving if that Lizzy Wizzy song she performed in the DLC is on. If not, I turn it off and try later...
The channel is random the first time I turn it on for the session, but always goes back to the same when I turn on the radio later, after having turned it off earlier (forgot the channel name now).
I might have a problem submitting these as my savegames are appearing as modded and thus likely invalidated by that for checking...
Never heard of it until now, but reading through it it sure sounds annoying as heck, especially as it's a persisting bug? Oof.

It's so common for me that I always buy the highest-tier versions of the attachments every time I see them at a weapon store just so I have extra on-hand when the downgrade happens.
Fix: Save/reload the game
Not familiar with any controller issues with Trauma Drama. Sure it's not a stick drift issue that can be fixed with deadzone settings?
I have never heard of this issue and also not found any other threads or video detailing or documenting this. For further investigation, does using a mod like unequip mods revert weapon mods to their original state or not?
Added to first post. As I've never experienced this bug myself, does completing one of the Cap's car missions also fix this?
- Reverb audio effects nolonger work (the echo effect when driving through a tunnel, or inside a parking garage), instead your engine sound just becomes more muted. No echo.
Fix: Don't know of one.
- After one of the game's early quests (i suspect it may be "Heroes", though not 100% sure), V will have a silent phone conversation with nobody (you can see your mouth move as if talking, but no sound) each time you launch the game, and get on a bike or in a car for the first time in that play scession.
Fix: Don't know of one.
- Misty can still bug out for the entirety of the "Fool on the hill" questline. She will stand in the meditation pose, facing the back wall of her shop, and cannot be interacted with at all.
This bug also prevents you from talking to her about the new murals you see in Dogtown.
Fix: If you find all the murals related to the "Fool on the hill" questline, she starts working again.
- Durring the last mission of the "The Prophet" questline, it is possible for Johnny to get stuck after speaking to you, and he will be standing there, just outside Misty's shop, for the rest of the game.
Fix: Don't know of one.
- Mitzutani Shion MZ2, when drivin by AI, and the AI decides to honk their horn, the horn will get stuck and keep blaring forever abd ever and ever.
Fix: Kill every single Mitzutani Shion driver you see, kill them all, and not just the men, but the women, and the children.
- Mitzutani Shion MZ2, again. When the player steals one of these cars for a "Just another story" quest, the car's engine sound can randomly stop working. Only seems to happen durring these quests, and only for the Mitzutani Shion.
Fix: Urge to kill... RISING!
I might think of more later.
-sometimes the middle mouse button won't respond at all
- After the second johnny flashback, any implants in the unlockable slots that require perks are unequipped automatically.
- Some enemies will bug out and stop attacking if affected by cyberware malfunction. This happens 100% of the time to cyberpsychos with shotguns. Not sure if normal enemies with shotguns are affected.
- Vic does not sell Bionic Joints.
- While waiting for Raffen Shivs to arrive in Ghost Town, waiting will delete all your timed buffs like the ones from sleeping and showering.
-Tier 3 bionic joints have better stats than the tier 4 version.
We all probably need to do a round of submitting bug reports each.
