Version 2.12a list of bugs & fixes
Since the game is now seemingly in maintenance mode and will likely only spontaneously receive updates (if at all), I thought it might be a good idea to bundle the current list of bugs and possible fixes.
Have compiled a list of things I came across during my current (modded) run, with potential causes and fixes.
If you happen to know more bugs (and fixes), please add them below! Might also be a good idea to pester CDPR's support every now and then with those so they don't get too comfortable leaving the game as is. :>

Quests
Fool on the Hill
- Misty can still bug out for the entirety of the questline. She will stand in the meditation pose, facing the back wall of her shop and cannot be interacted with at all.
This bug also prevents you from talking to her about the new murals you see in Dogtown.
Fix: If you find all the murals related to the "Fool on the hill" questline, she starts working again.

The Prophet
- During the last mission of the questline, it is possible for Johnny to get stuck after speaking to you, and he will be standing there, just outside Misty's shop, for the rest of the game.
Fix: Don't know of one.

Balls to the Wall
- Paco Torres is both beheaded and sending a pic from Kenia after using the Aldecaldos option to get them out, granting both iconics. Truly Schrödingers Paco Torres.
Fix: None

Waiting for Dodger
- The NCPD car will run over one of the goons upon getting the two officers out of the situation, putting the remaining Barghest troops (including Dodger) into combat mode.
Fix: None, but combat can be avoided by running out of the garage and leaving the area.

The Highwayman
- The only unmarked quest in the game, and the only one without quest markers
Fix: None, look up a guide if needed

I really want to stay at your house
- Judy will break and cannot be spoken to in her apartment until the quest has been completed. Also affects mods like Romance Enhanced. Probably the same for all romances.
Fixes: Finish the quest and don't initiate it again via text message.


Gameplay
- Small change introduced in 2.11 to make the Synaptic Accelerator cooldown start after the buff instead of at the same time happens to cause the buff to continue to repeat as long as an enemy is detecting you. (taken from the mod's description)

- No Wanted Level - Your wanted level can permanently bug out, showing no wanted level and also not increasing it. This bug is persistent through save games.
Causes: Potentially linked to changes made to police systems with Phantom Liberty?
Fix: No permanent fix. You can "fix" a playing session by going through the Corpo Plaza Apartment scanner. This will revert again when loading a save.

- Tier 3 bionic joints have better stats than the tier 4 version.

- Vik does not sell Bionic Joints.


Other
TV
- News segments have no audio since patch 2.1x (possibly earlier)

- Adverts can interrupt one another

Audio
- Reverb audio effects nolonger work (the echo effect when driving through a tunnel, or inside a parking garage), instead your engine sound just becomes more muted. No echo.
Fix: Don't know of one.

Radio
- Radio cuts out every now and then (not counting the radio turning off when entering quest areas). Seems to happen more often when on foot.
Reasons: Possibly linked to synchronizing vehicle and on foot radio, or linked to quest zones.
Fix: None

- Radio being lower volume when using first person view in vehicle
Fix: None

- Radio returning wrong channel when turning back on. (Tested with Growl FM -> Vexelstrom, untested with other stations)
Reasons: Possibly linked to quest status updating, or returning wrong value when turning radio on
Fix: None

Phone / Menus
- Rare bug can permanently disable your phone (it appears red / unusable). Same with radio.
Causes: Unknown but usually happening in quest areas or during dialogue, and noticed afterwards.
Fixes: Go to one of the Joytoys and let the scene play out, it'll unstuck the phone.

- when closing menus there's often a blinding wall of bright fog in the player's view that slowly fades. Not sure what causes it exactly but can be very distracting.
Fixes: None

Cars
- Car does not summon no matter what car you try.
Fix: Save/reload the game

- Mitzutani Shion MZ2, if AI decides to honk their horn, the horn will get stuck and keep blaring forever.
Fix: Kill every single Mitzutani Shion driver you see

- Mitzutani Shion MZ2, again. When the player steals one of these cars for a "Just another story" quest, the car's engine sound can randomly stop working. Only seems to happen during these quests, and only for the Mitzutani Shion.
Fix: Finish the Quest


Locations
- Aldecaldo new camp bed still not usable (has been reported since 1.6x)
Causes: Incorrect marker set or overlap with wardrobe
Fixes: None

Many thanks for this! I'm going to try finding that "The Highwayman" mission :letsgo:

Regarding "Balls to the Walls", I chose the Aldecaldos option both times I did that, and got the post cards some time after driving them to safety. But I don't remember anything about beheading? Can you find him beheaded somewhere in the game later, perhaps?

And good tips about the fixes for the missing tv-audio. Been using those since they were released. Wonder why this hasn't been fixed 'officially'. Maybe there is some censorship involved, or something? That they're no longer allowed to keep those 'news-stories' in the game? No idea, though.

The easiest way to "start" the Highway man is going to the dam area (where you do the drink / shoot quest for the 6th), on the south eastern corner on the hill is a building with a police car parked in the corner. There's a garage door to open.

You can find Paco's beheaded corpse across from Dogtown entrace, on the upper area of the NCPD building, an iconic sniper lying beneath him.

If the news weren't allowed to be kept ingame they would have taken them out entirely. Probably a QA oversight when fixing something else. Thankfully, fix works.

I don't know of any radio fixes but if you do, let me know.

I'll look for Paco. Could be I've already found him without connecting the dots.

I don't use the radio much, just check from time to time when driving if that Lizzy Wizzy song she performed in the DLC is on. If not, I turn it off and try later... :steamsunny: (or there are a few other songs on that channel I may listen to). And I drive a bike in 3rd person. So I haven't noticed those issues.

The channel is random the first time I turn it on for the session, but always goes back to the same when I turn on the radio later, after having turned it off earlier (forgot the channel name now).

Added a couple more bugs and possible solutions (most prominently, the phone getting unusable bug).
I might have a problem submitting these as my savegames are appearing as modded and thus likely invalidated by that for checking...

even though they said it was fixed, i still get the bug where i can't get wanted levels and it only occurs after doing PL quests. probably after the first quest "dog eat dog".

Never heard of it until now, but reading through it it sure sounds annoying as heck, especially as it's a persisting bug? Oof.

My HUD still randomly vanishes.. I also wonder if the annoying bug in Trauma Drama where your character shoots up by default with a controller is still an issue.

There is a gameplay bug where certain weapon attachments will downgrade to their lower-tier versions when a major gameplay change occurs (like "no weapons" portion of a quest). I notice this most often with the CQB scope and the CS-1 Taipan suppressor.

It's so common for me that I always buy the highest-tier versions of the attachments every time I see them at a weapon store just so I have extra on-hand when the downgrade happens.

Haven't really found any other reports of it for 2.12... does scanning things fix the HUD?
Not familiar with any controller issues with Trauma Drama. Sure it's not a stick drift issue that can be fixed with deadzone settings?

I have never heard of this issue and also not found any other threads or video detailing or documenting this. For further investigation, does using a mod like unequip mods revert weapon mods to their original state or not?

Added to first post. As I've never experienced this bug myself, does completing one of the Cap's car missions also fix this?

v2.12/2,12a bugs to add:

- After one of the game's early quests (i suspect it may be "Heroes", though not 100% sure), V will have a silent phone conversation with nobody (you can see your mouth move as if talking, but no sound) each time you launch the game, and get on a bike or in a car for the first time in that play scession.
Fix: Don't know of one.

-sometimes when trying to steal a vehicle the prompt will glitch out and not allow you to enter any vehicle
-sometimes the middle mouse button won't respond at all

I have encountered another bug: After the prologue, when you have to meet Jackie at the doors of the V's megabuilding, you have to talk to Jackie, then he will talk to you about the Deshawn new mission, if after the conversation, instead of driving V's car you drive for example some car u can buy by then, you will see some buggy Jackie in the car, but the real bug is, if after doing that you try to enter V's car again, you will can't drive it, you can't do anything at all, and reloading a save after doing that, won't fix it.

What ive found :

- After the second johnny flashback, any implants in the unlockable slots that require perks are unequipped automatically.

- Some enemies will bug out and stop attacking if affected by cyberware malfunction. This happens 100% of the time to cyberpsychos with shotguns. Not sure if normal enemies with shotguns are affected.

- Vic does not sell Bionic Joints.

- While waiting for Raffen Shivs to arrive in Ghost Town, waiting will delete all your timed buffs like the ones from sleeping and showering.

-Tier 3 bionic joints have better stats than the tier 4 version.

We all probably need to do a round of submitting bug reports each.
