Item Quality Degrades After Reload

I've noticed that item quality degrades in quality every time you reload a saved game. I they made this change to combat save scumming. I've got two questions.

1) is there a way around this?
2) is this change documented somewhere in the patch notes? I've tried looking but I can't find anything.

It does not, if this is happening, that sounds like a bug.

I've noticed it several time during a mission. I'd kill a bunch of mobs and if I don't like what I get I'll reload a previous save eventually I'll be down to white items.

I haven't reloaded a game because I wasn't happy with the loot, but if it really happens (looks like it) it could be intentional, to discourage people from reloading to get better loot?

Not sure, though.

If you are talking about the quality of items the enemies drop, then it is not degrading, the quality tier is random.

After save reload never, at least never happen to me.

After putting Iconic weapon to stash - very often attachments tier downgrade, my workaround are simply detach any attachment from Iconic weapons before i put its to stash. And yes, that happens only with Iconic versions, not with regular ones.
Very very rare happen oposite - attachment tier become upgraded after putting in stash, be this way i got Legendary tier CS-1 Taipan silencers etc.. But this "upgrade" happens extra rare. Nine times frorm ten Tier 4 attachment downgrade to tier 1 (white).
