Depending on your Ram availibility (Intellegence, Perks, Buffs, Cyberdeck) you have so many spaces to put the characters (Numbers or Letter Sequences).
So lets go with the 1 line like for breaching. Say it requires 4 Characters and you have 6 Spaces.
So the Sequence you need is lets say 1C 55 1C But the top row (across) only has 55s, E9s and BDs. Look for a !C in a collumn (down). Since the timer isn't running, make sure from that 1C that you can link to a 55 in that row (It goes Row, Collumn, Row, Collumn, Row; get it) and that 55 needs an IC in that collumn. So in the example I would select whatever Character in the Row will lead me to the IC in the Collumn that links to the 55 in that row which will link to the IC in that collumn.
As long as you can get the required Sequence in order within the space allowed, you are good. When you get to the Link in ones that give money and Quick Hack Componants it gets more involved as you will need to chart out more than one sequence code (Recommend a Deck with more Ram.)
Honestly watch some videos, It took me a while figureing it out on my own, I get Mass Effect Andromeda Sudoko Flashbacks from this stuff. God I hated those puzzels, even worse that ♥♥♥♥♥♥♥ run to the exit to escape the Purge. Never did finish that game and figure out what the hell was going on.
Although it does feel a bit weird that you haven't been able to see a chain in 10 tries.
The first is chosen from the top row, horizontally. The second is chosen from that vertical column. The third is chosen from the horizontal row you just went to, and so on - alternating between vertical and horizontal.
Sometimes you simply won't have enough inputs to complete all three combinations, but you can usually get one or two. I tend to aim for the third (and most profitable) code, then see if I can tie in the 1st combination as well with the space I have left.
The timer doesn't start until you click something on the grid, so feel free to take the time and plan your route before clicking. :-)
Edit: if you mouse-over the combinations on the right side, it can highlight the individual bits for you, sometimes making it easier to plan. Like if you need E9, but can't see a way to do it with the one you have, highlighting might show you that there were actually 3 but your own eyes (irl) didn't catch them.
This has helped me out a couple of times.
Edit2: I've added a separate comment with the video that taught me how to do the hacking minigame. Skip the first minute of the video.
You can ALWAYS do it - Whether or not you have a buffer long enough (Upgrade your Cyberdeck) to actually get more than one hack off is another matter.
You don't have to start on the right cipher. You just can't deviate from the pattern once you've hit the first cipher in a pattern.
If the hack starts with a 55, you can do whatever UNTIL you hit a 55. Then you need to follow the pattern till the end or it fails.

I assume you have an OS installed that doesn't allow hacking. Such as the berserk or the sandevistan.
