Time is pretty elastic in this game. V will not run out of life before he/she runs out of things to do. There's no timer. It's one of the things you have to suspend your disbelief over.

I feel that the gameplay mechanic was understood and the question was asking more for just how V might actually react if there really was a time limit on how many in-game days V can play.

The first time I had V get the egg, it hatched without me realizing how many in-game days I had played, and I had a lot of content left to do. The last time V got the egg, I did as much as I could prior to meeting Takemura and ran out of things to do before the egg hatched even with the added PL content. A hard time limit would be frustrating and stressful, but possibly appropriate and easily do-able without missing anything.

The "porcelain ♥♥♥♥♥" and "go with Rogue" options are always possible (along with the non-ending), but what if the player took 6 in-game months? Maybe the only options would be Rogue (and the non-ending) and skipping meeting Hanako altogether with Johnny still dragging V to the ripperdoc. (Of course, any other opened options would still be available, but the minimum would no longer include Hanako's offer.)

...or maybe it already does that and nobody's played that long, yet? (Doubtful, but I've never looked for that information.)

E: V can't place the egg into its bowl to start its incubation timer until after meeting Takemura in Playing for Time.

It does not track, no.
Folks who stood around, fast forwarding time 24hours over and over, outside clothing vendors would have complained about it otherwise lol.
(used to be, they would get a relic malfunction every time they did that, but I think those were yeeted out with the rest of the unscripted malfunctions)

In character......

Well, I decide which if my friends get my wheels.
Rogue gets my red Yaiba Kusenagi, of course.... And Kerry gets that sweet little bike I found up in Watson, the one the Tygers lost....
I drive around four of the dirt road cars out to Panam..... The Beast, that Colby I found after helping Mitch with Scorpion.....the Aldecaldo 4 wheel drive that Dakota gave me....along with Venom (that's the name I gave Scorpions bike, the Apollo)...after all. It stays in the Family.
Oh, and that handy pre loaded smg ready car I found when chasing that missing Delamain over in Northside.
Then there is some charity work....try to give every veteran some cash at least once....
Visit my soon to be neighbours in the columbarium, chat with Jackie on the holo....he never calls back, but hey, it's nice to leave him a message or two....
Take pictures in some of NC's beauty spots.
Hang out with my chooms, Panam and the Carldos....River and the kids, or watch Kerry play tunes....
Time goes past kinda quickly, before you know it, your flatlining on some bench, broseph in hand, over in Pacifica...watching your last sunset.
