It made a lot more sense in the initial release of the game, but since they completely changed the economy of cyberware, and how it works in terms of upgrades etc etc with the 2.0 update, it now really does seem like Vicky is taking you for a ride.
They changed the economy, but they didn't change this quest to match it.
Jenkins' Tendons by far the worst in my opinion
With that in mind, Viktor is giving you the best of what he has. May not be the best on the overall market, both legal and not, but ripperdocs certainly don't screw around when paid. As for the 20k eddies, that is all from previous implants and maintenance V did offscreen, s/he just owes Vik.
The reason why all this isn't obvious in the game is because, well, it's a game.
Not to mention, in Cyberpunk lore, Kiroshi is the best optics company in the world
He also wipes and cleans out the virus you catch while he's at it.... That probably accounts for some of the costs too.
- gets you Kiroshi optics, the top of the market, the Glock of eyeballs
- no interest, no time limit, no collateral? That's a pretty dang good "loan".
- NCPD database access, that thing earns me that 20K in half an hour and is awesome profit after that.
Vik is the best ripperdoc in the game and actually good person.

Why would he when it was a debt? He even offered to let us keep the money (the game doesn't allow us to take his offer, but still).
Well, I thought it was kind of nice of him to pull the bullet out of our skull, save our life and take care of us for weeks. And he didn't even demand payment for it.
He could have been someone like Fingers or Bucks.
Once again: This quest was written back in the day when there was only one Kiroshi implant in the game.
Patch 2.0 added a whole slew of new, and much better, Kiroshi optics for you to buy, so it nolonger makes sense... but it did before patch 2.0 was released.
I guess nobody thought to check this quest, and if the pricing of it still made sense after they completely revamped how implants work, and how they are priced.
Jokes aside, these actions make him a friend, indeed, but that's another event.
