The second explanation is that Johnny had some control over V's body at that moment and if you pay close attention to the scene after that Johnny finds out that if he moves his hands, V's hands moves the same way as well, Therefore maybe Johnny doesn't actually slap V, V is slapping himself/herself because Johnny wanted it.
Even if it's a hallucination, if you experience it as real, your body will still react as if it's real.
As time goes on, V gets more used to sharing space with him. But right at that point, they have never had to be stubborn with him.
After he tries to remove the chip, he also is the one banging Vs head against the window, that's not us the player or V.
There are a couple other instances where he can sort of 'touch' V. Tho thankfully most of them are a lot more friendly. He'll hang onto V or grab V's arm, hand or shoulder.
But again, these are points where V is weak (for a moment)
But you are easily distracted and ♥♥♥♥ can go downhill much faster.
He probably doesn't - in the sense that he does the movement of slapping but can't actually hit V since he's not there. However, since V's brain is convinced Johnny's there, it's making V's body react as if Johnny had slapped V. At least that's how I explain it to myself.
