I just passed this section of the game last night and found it easy to sneak up on her. She instantly aggroes sometimes if you were fighting openly prior to getting to her though. As for the sneak attack, if it doesn't show up for you on some enemies it means you are too close. It's needed fixed since forever, but backing away a couple steps will typically give you the prompt.

Distance wasn't the issue, I've only ever had this problem today and I've been playing this save on and off for about a week. There was another enemy I encountered before her that had the same problem where no matter the distance it refused to allow a stealth take down. With the melee being bugged for me I will also take damage mid air (double jumped up) from a charged punch from an enemy, which to me makes no sense. I also had Matilde ground slam me directly and it did no damage. I also went undetected the entire way until I got to her and had the sneak attack prompt only 2 times out of like 10 save reloads.

My frames are fine, not too sure how that would really effect the situation either. When I fought the twins I actually ended up having to just sprint away from them when I saw their charged attacks because no matter when I used dodge they would still hit me from like 5 feet away. Sprinting away would still sometimes get me hit with them being far but it seemed to work better than the dodge function for me.
