Could not tell you about the DLC; price of that is crazy high for what effectively unlocks a sub region. So I skipped it.
So $30 is too much for an expansion that takes 20 +/- hours to playthrough? Really? There are a lot of full priced $60 games that has less gameplay hours.
NPCs easily suicide against fire(walking through burning cars or side-dashing into a burning garbage), getting stuck in weird positions(hovering while sitting on an invisible chair, in the middle of the road, or sink into walls while smoking), act weird when driving(last time I've got run over by an NPC who decided to just give up on using pedals, resulting in its car rolling down the hill and giving me a good slap from behind). There are odd "glitch squares", missing/misplaced props in some areas around the map, some props or parts of buildings "ceasing to exist" from certain angles of view.
Maybe you were talking about different kind of bugs, but the bugs I've seen are pretty mild. From what I understand, they've fixed most gamebreaking ones.
Though, if you're seeking for "perfect immersion" - forget about it.
The 'Open World' is still pretty iffy at times, civilians vanishing when you turn around, walking into your vehicle when you are parked and then running around in a panic sometimes triggering a wanted star if they then fall over, 'cutscenes' were your driver just runs over a bunch of pedestrians, or they phase through the car. There are still distant traffic jams in the desert, long trains of cars that just vanish as you get closer, it feels forced and not really dynamic (it's better than it was but not by much).
It has gone from a very below average game that really wanted to be future dystopia GTAV, to an average game. It's worth a play-through if you can pick it up with the DLC in a sale, but don't expect it to be your 'main' game as it has far too may flaws.

Considering I got the base game for half off. $30USD is too much for a standard open world DLC; and always will be.
There is nothing groundbreaking here (as in the DLC; its more of the same game play) to justify that asking price.
Like you I got a refund at launch because of how utterly disappointing it was, but now post-2.1 I'm on a 2nd playthrough with mods.
The free play outside of the main story feels like a poor mans GTA, but it's still good.
MINOR SPOILER:
The story is still paradoxial in the sense that you only have a couple of weeks to live after the start of the main ordeal, yet you "should" spend time exploring, do side-missions, and now also the Phantom Liberty DLC. It's quite silly and they should have changed this narrative but it is what it is.
It's not "WOW! This is amazing!" Like GOW 4 or 5 (Ragnarök), but it's ok.
I'd say buy it at 50% or discount or more.
For me its been worth playing since day 1 ..through all the changes, The only way you're going to know if its for you is to try it yourself!
Many mods are actually cut out content that has been reintegrated, better gameplay mechanics, E3 hud/weapons, excellent reshades/ luts, additionnally modders created clothes and equipments, new HD textures etc...,
The game has tons of possibilities now with a infinite gameplay duration.
