Ideally, I wish that I could skip Act 1 entirely, but not in a simple skip but a complete removal of the relic. Not possible of course, since Act 1, Johnny, and the relic are hard coded into this game.
Your best bet would be to go through Act 1, the start of Act 2, don't meet Takemura at the diner, and just go full-on sandbox mode. The main story won't go anywhere, so there's no need to rush to complete it. You just have to get through the first Act.
(I do this too. Twice for each V I play rather than just once tho)
Make one V per lifepath you like. (Per body type too, of you play both or a combination of)
Play thru the first act on easy or normal, ignoring everything but the main story
Don't use any level up skill points or invest in any perks (that's why I say do it on easy lol)
(So, grab your starter cyberware. Go talk to Dex. Go get the Flathead. Talk to the client. Visit Afterlife then do the heist)
Start act 2 in the shower.
Make a save
(if you use mods, you could use the 'named saves' mod like I do, to let you know exactly which save is which. If not.... Then make a rememberable thumbnail. I face the vending machine to save 'post heist' saves for the thumbnail picture)
Then you just remember to never delete those saves with the vending machine thumbnail.
(Make a note of the save game number. Head to your game save location. Make a copy of the 'manual save # number' someplace safe, outside the actual save game folder)
Use these as your starter saves.
You can change your hair, makeup and eye color at the mirror. Your skin tone, face shape and tattoo's at the ripperdoc.
Remake your V mostly that way, so if you want to be a different looking nomad or corpo to the base one, you can.
You can then invest in your attributes and pick your perks.
Head out and play the rest of the game without having to go thru the heist again.
Change the difficulty to your preference (since playin on easy was to help you get here without using your attributes points or needing to invest perks early. Let's you build how you want to, once you are free in act 2)
