I'm unsure whether i should get it asap (Started 2nd playthrough on Very Hard. Level 6/7. And just finished talking to Judy and Evelyn...

Get Guts as soon as possible, it is one hell of a boomstick! Then just keep it upgraded. Make it even better by leveling up your shotgun abilities in the BODY skilltree.

If you really want to use it, you can go get it any time. I would take my time though, rushing for things always feels off to me.

When is the appropriate time to pick up Guts after gaining access to the whole map? It just sits there.

Mot sure if it's a good weapon on VH early on. Without substantial Body and some Perks the recoil is so bad that it seems to take forever (to me) to bring it back to target. As with most gun-related things, YMMV.

Whenever you naturally find yourself in the area. IE: for quests, tarot card, exploration, whatever.
